To see one of our doctors you need a referral from your GP or referring specialist.
Details required at the time of making an appointment include:

- Your date of birth
- Your address
- Your phone number
- Name and provider number of the referring doctor
You need to bring to your appointment:
- Your referral letter
- All relevant investigations you have undergone
- Scans and x-rays
